Required Documents are:

• A medical certificate from the competement medical committees at the ministry of health in the Kingdom.
• Power of attorney to someone in the Kingdom of Bahrain in the supplement registration procedures on his/her behalf, if any.
• To be at least 16 years of age and not more than 50 years.

• Copy of Marriage certificate (If she’s married).
• CR Copy (If he is an employer).

• Copy of Birth certificate for eligible children (Female).

Requesting to transfer pension fund
Required Documents are:
• A Copy of the new IBAN account number.
• A release letter from the old bank(recent copy).
Salary amendment
In case of salary amendment from beginning of the year Required Documents are:
• Employment contract with effect from January .
• Pay slips from January.
• Bank statement from the beginning of the year until the current period (Incase, the employee salary is not transferred to bank, the employer must acknowledge in the form that salary are not transferred through the bank.).
In case of salary amendment from date of enrollment Required Documents are:
• Employment contract with effect from date of enrollment.
• Pay slips from the date of enrollment.
• Bank statement from the date of enrollment (Incase, the employee salary is not transferred to bank, the employer must acknowledge in the form that salary are not transferred through the bank.).
In case of salary amendment from beginning of the year before the salary due date Required Documents are:
• New employment contract.
• The employer must acknowledge in the form that the employee will receive the updated salary through Bank transfer or cheque or cash.

Work injury
Required Documents are:
• Occupational trauma / disease report.
• Emergency service report from salmaniya hospital or bdf hospital.
• Medical certificate.
• Traffic report for road traffic accident .
• Death certificate and all the documents related for death reason.
